She constantly worries about annoying, boring, or offending others, going as far as asking Hajime Hinata if it would be okay if she remembered his name when introducing herself. This delicate persona makes her a natural target for Hiyoko Saionji's predatory and cruel personality. However, as the Ultimate Nurse, Mikan tends to act much more confident when doing her job, and she is knowledgeable and intelligent. Her medical knowledge proves useful during assassinations, as she provides autopsy reports including information on the corpse's condition, details on injuries, cause of death, and time of death. Furthermore, she occasionally gives (often unwanted) health advice to others regarding things like eating and sleeping, and can occasionally begin to describe things like bodily functions in too much detail in an attempt to provide other students with information on the matter at hand. Unlike most students, Mikan has even mentioned that she enjoys being on Jabberwock Island because her classmates acknowledge her and her talent makes her useful. Perhaps due to her high understanding of human physiology, Mikan is shown to be very skilled at reading people's faces, but she tends to overanalyze them and is quick to assume that she is disliked by those around her. She generally wants to please and help other people, but her overall lack of confidence and desire to not be a burden causes her to rarely assert herself or openly express her feelings; if she feels the need, she will likely ask her peers for permission to do so.

This is greatly exacerbated during class trials, where despite her talent being incredibly useful for solving murders, she rarely ever speaks up of her own accord. Since her social intuition is generally poor, if there is an awkward pause in a conversation, she will immediately panic and apologize, believing it to be her fault. However, despite lacking many basic social skills, Mikan does actively try to befriend her surroundings, changing aspects of her behavior to match what she projects as the thoughts of her peers. Notably, she appears exaggeratedly clumsy and often ends up in embarrassing and provocative poses, causing her to apologize a lot, though Hiyoko has clearly hinted and even pointed out that Mikan does these poses intentionally to gain attention and pity. Due to her persistently clumsy and cloying personality, even with all the pity and indulgence she usually receives from her kinder classmates, many of them still find her somewhat off-putting, incompetent, or annoying, causing them to distance themselves from her rather than treat her openly poorly. Due to the abuse she has suffered in the past, Mikan is very awkward and has a distorted perception of both positive and negative social interactions. When treated with any individual respect or sympathy, her first reaction is most often outright confusion, and sometimes an assumption that she has done something wrong. When she first befriends Hajime, she invites him to beat her up, strip her naked, draw on her, or do other bad things to her, and states that she has done this many times to previous classmates so that they will not hate her and continue to interact with her.
