Izumi is of average height, with a slender build and pale skin. He is noted to be really handsome and attractive with long eyelashes and feminine features, which is often pointed out by others. He has blue eyes, messy black hair that reaches the back of his head and thin eyebrows. Izumi has a total of nine piercings: four in both ears and one on his lip, all of which he did himself. He also has several tattoos on his left shoulder, left arm and left side. Initially, Izumi wore warm clothes to hide his tattoos. He kept his shoulder-length hair straight to hide his piercings. He would get a safety pin piercing when he felt that school was giving him a hard time. It has also been suggested that he had suicidal thoughts. Upon meeting Koichi Shindo, who was Makio's friend at the time, Koichi treated Izumi much better than his classmates, causing the two to become good friends. The two continued to communicate even after they went to different schools. Izumi credits Koichi for helping him change. Going into high school, he still retains his dark aura, which is pointed out by his classmates. When Izumi and Kyoko meet each other's hidden sides, Izumi displays a friendly and laid-back attitude, a trait that Kyoko did not expect in him.
