Smoothie is a woman of gigantic proportions. Her eyes are blue and detailed, and her lips are prominent. She has long, white, wavy hair that covers her right eye and extends down her back. On her head she wears a large beige cap that covers her ears (similar to a beaker) adorned with red circles on its sides.[3] Her attire consists of a leotard of sorts with stripes of various shades of pink, which she wears tight around her body under a long, wide yellow scarf or blanket wrapped around her neck and covering her breasts. Her elbows are covered by thick beige armor in the same style as her cap. Since she does not wear pants or a skirt, her thighs are bare, sporting a tattoo of a thorny rose on the right. She wears a pair of boots in which pink and purple predominate (similar in shape to Cracker's) that extend past her calves to reach her knees, in what appears to be a sort of armor decorated with a pattern of diamonds and circles. During the tea party, she did not wear her cap and left all of her hair uncovered.

Smoothie appears to approach her goals with a carefree and passive attitude; agreeing to help people as long as it doesn't prevent her from completing her other duties. Despite her attitude, she will regret it if she fails the task at hand.[8] She often employs a wry tone when speaking, such as when she commented that it "seemed" like the Straw Hats were being "harassed" during their chase.[9] She likes to squeeze the life out of people and animals despite the pain it causes them, and enjoys drinking the liquid produced; a somewhat sadistic method.[3] She even often offers said liquid to third parties; especially during tea parties. Apparently, the more unique or beautiful the person or being she squeezes, the better the taste,[10] which is why she attempted to squeeze Nami after the ceremony went to chaos.[11] Despite her carefree attitude, Smoothie can become very serious if the situation calls for it, choosing the most effective and quickest solution to alleviate the problem. She is also very perceptive about the nature of other people, as shown when she neglected to inform Big Mom about the prisoners' rebellion at Whole Cake Chateau and thus avoid her mother's furious anger. [12] On special occasions, such as a tea party, she is shown to be friendly and polite to the guests; greeting them and warmly offering them freshly squeezed juice. [10] Smoothie appears to have great self-confidence, telling her brother Perospero that if he had given her the task in the first place, all the problems would have been solved.[13] When she tackles a goal, Smoothie focuses all her efforts on it and does not care about the consequences of her actions; such as when she assumed a gigantic size to sink the Thousand Sunny at the risk of sinking her own ship due to her size and weight.
