Garou is a young woman of average height with sharp features and white hair that stands up in two elongated spikes with yellow eyes. Despite her apparent thinness, she is shown to have a muscular body with quite turgid curves. She wears a tight black shirt and loose white pants, similar to her former master Bang, with a yellow belt wrapped around her waist. She has G-cup breasts, a round and firm ass, large and marked thighs.

Garou is portrayed as an evil character with a hatred towards heroes and has a confident and vengeful characterization. She hates being insulted and will attack anyone who offends her. She thinks highly of herself and uses that claim to threaten high-ranking personnel such as heroes and Sitch. She will also attack low-ranking heroes and Hero Association personnel if she encounters them. Garou hates being popular, as she believes that the popular one will always win in the end. This is because as a child, she had no friends and the popular kids at school took pleasure in abusing her, both physically and verbally, particularly one boy named Tacchan. She would be forced into playing Heroes, where Tacchan ("Justice Man") would beat up Garou ("the monster"). Garou noticed how everyone liked Tacchan, but hated her. After seeing her life repeated over and over again on the Justice Man TV show, she developed a distaste for heroes in general and believed that in the end, most will always want him ("the monster") dead. Although Garou is a villain and is considered evil by most, she does possess some sense of morality as she fights heroes in ways that defeat them without killing them, though she does not seem to have a problem with other individuals killing them. She has a soft spot for children, as shown when she converses with Tareo in the park, or when she stops fighting Metal Bat after Zenko appears, whom she protects from Sludge Jellyfish (in his attempt to kidnap her). When Tareo calls her "Miss" after Garou told him not to, she does not get angry at the boy. She later rescues and protects the boy from the Monster Association, even risking her own life to do so.
