Once upon a time, two races ruled the Earth: humans and monsters. But one day, a war broke out between them, and humans won. Seven of the greatest human magicians locked the monsters underground with a powerful spell - a barrier. The only entrance to the Underground is a small hole in Mount Ebott. Many years after the end of the war, a human child falls into this hole, which the player controls. There, he meets Flowey, a flower character who explains the basic mechanics of the game to the protagonist and offers to increase "LV" or "LOVE" by earning "EXP" by killing monsters. When Flowey tries to kill the child and take his soul, the protagonist is saved by Toriel, a monster who introduces herself as the guardian of the Ruins. She teaches the protagonist how to solve puzzles and resolve conflicts with enemies peacefully. She treats the protagonist with motherly love and wants to protect him from Asgore Dreemurr, the king of monsters. The protagonist's main goal is to explore the world of monsters, get to know its inhabitants and eventually find a way out of the Dungeon. The protagonist leaves Toriel in search of Asgore's castle, which contains an exit to the Surface. During his adventure, the child meets various monsters, the most significant of which are Sans and Papyrus, the sentinel brothers of the Snowdin forest, Undyne, the head of the Royal Guard, Alphys, the royal scientist, and Mettaton, a robot TV presenter created by Alphys. The protagonist has to fight some of the above-described characters. The player can choose to kill them or show mercy. In the latter case, the hero can become friends with them. As the game progresses, the protagonist gradually learns about the reasons for the war between monsters and humans that has broken out, and that, as it turns out, other people have fallen into the Underground before him. Asriel, the son of Asgore and Toriel, befriended the first fallen person, who was accepted into the Dreemurr family. However, the child committed suicide by eating poisonous flowers so that Asriel could absorb his soul and pass through the barrier. When Asriel reached the human village, the villagers attacked him in rage and fatally wounded him. King Asgore decided to end the suffering of the monsters - he gave the order to kill and take the souls of every person falling into the Dungeon in order to destroy the barrier and take revenge on the people. To do this, he needs to collect seven human souls, and to complete his plan, he lacks one soul - the soul of the protagonist. Further developments depend on what decisions the player made when confronting the monsters. The game highlights three main paths of plot development: Neutral path - obtained if the player killed some monsters and spared some. Before arriving at Asgore's castle, the protagonist learns that he needs the soul of one monster to cross the barrier, after which he enters into battle with Asgore. Before this, Sans explains that "LOVE" is actually "Level of Violence" and "EXP" is "Execution Points" and not experience, as the player might think. The hero fights Asgore, but they are interrupted by Flowey, who kills Asgore and absorbs human souls. With the help of the rebellious souls, the hero defeats Flowey and appears in a room with an exit to the Surface. He can also decide what to do with Flowey: finish him off or spare him. After leaving the Underground, the protagonist receives a call from Sans some time later, telling them what happened to the Underground after Asgore's death. The epilogue has many variations depending on who the player killed and who they spared. True Pacifist Route - Developed if the player avoids killing monsters. During the game, the protagonist tries to befriend everyone, using various actions and, in some cases, flirting. By the end of the story, the protagonist learns that Flowey is Asriel's reincarnation, created as part of Alphys's experiments. Before the failed battle with Asgore, Toriel and the rest of the monsters the protagonist befriended decide to intervene. However, Flowey ambushes them, taking all the human and monster souls to take his true form as Asriel and fight the protagonist. During the battle, the human manages to save his friends and ultimately win. Asriel transforms into a child form, destroys the barrier, and expresses his remorse before leaving. The protagonist falls unconscious and wakes up to find himself surrounded by friends. The player learns the protagonist's name, Frisk. The monsters head to the Surface and start a new life. The player can also decide Frisk's fate: stay with Toriel or go home. The Genocide Path is achieved by deliberately engaging monsters in each region of the Underground and killing them until they stop appearing. When the protagonist enters the Last Corridor before Asgore's castle, Sans tries to stop him but dies, and the child heads to Asgore's castle. Flowey, in an attempt to atone, kills Asgore, but is himself killed. After this, Chara, the first human to fall, appears. He explains the reason for his reincarnation and gives the player a choice: destroy this world or refuse. However, he destroys the world anyway. To return to the normal game, the player must give Chara a soul as payment for restoring the world. Repeated playthroughs of the Genocide Route, as well as the True Pacifist Route, will have permanent changes.

During his adventures, the child meets other monsters, the most significant for the plot are Sans and Papyrus, the guard brothers of Snowdin Forest, Undyne - the head of the royal guard, Alphys, the court scientist and Mettaton - a robot TV presenter, actually created by Alphys. The hero has to fight some of the above-described characters. The player can choose to kill them or show mercy, in the latter case, the child can become friends with them. Azriel, the son of Asgore and Toriel, befriended the first human child, who was also adopted by Azriel's parents. However, the child became terminally ill, and his last wish was to "see the flowers of his native village." Azriel absorbed his soul and was able to pass through the Barrier.
