Doflamingo is a very tall man (305 cm), even taller than other half-giants such as Kuzan.[20] His hair is blond, making him quite thin and muscular at the same time; his skin is tanned. He usually dresses in extravagant clothing. In reference to his animal theme, the flamingo, his clothes are brightly colored, with his signature pink feathered coat. Doflamingo wears orange sunglasses (purple in the anime). He also has an odd way of walking, walking in a bowlegged manner (although he does not always walk like this), which is also a reference to his animal theme, the flamingo. Doflamingo is somewhat less elaborately dressed than in his younger appearances, wearing a plain, open shirt that has a pair of Jolly Rogers on the cuff, a different belt, and pink-striped pants with white zebra stripes. However, he still wears his sunglasses which have changed from orange to purple (from purple to red in the anime). Even after his father took him away from his life of luxury on Mary Geoise, Doflamingo held on to his bloodline, believing that he still held the rights to his origins, complaining that he did not want to live inside a run-down hut despite being hunted, and continued to hold resentment towards his father to this day for taking him away from it all. Doflamingo's arrogance is a common trait of World Nobles. He hypocritically hates the World Nobles for rejecting him, but follows their same attitudes and actions, looking down on others for being "human" while considering his bloodline to be of the utmost nobleness.

Aside from his personal crew, the Donquixote Pirates, he also has allies in many places who always wear his Jolly Roger due to their business. As for his crew, it is evident that he does not choose anyone based on their age, as he is seen to have both young people like Dellinger and older people like Giolla and Lao G. The members with the rank of Officer of the Donquixote Family call him Young Master (若, Waka?),[24] although the Supreme Officers call him "Doffy", he treats all the members with the rank of Officer or Supreme Officer as brothers as they are like his family and will not forgive anyone who makes fun of them, showing anger when a subordinate could not hold back his laughter upon hearing Pica's voice.

Doflamingo is a descendant of one of the royal families that formed the World Government and moved to Mary Geoise. However, his father chose to stop being a World Noble and live a normal life with his wife and two children. Doflamingo was accustomed to his old lifestyle as a Celestial Dragon, and resented his father for giving up his World Noble title. Even though his father renounced his status as World Noble, the people who tried to live among them resented him due to his history of hatred towards World Nobles. When Doflamingo was eight years old, the people of the city finally attacked them and burned their mansion, leading the Donquixote family to live in a makeshift pit in a pile of scrap metal. After a time, Doflamingo lost his mother to illness, believing that his family's new deplorable lifestyle was the cause.
