about the virus:

As the disease spreads, its ominous symptoms manifest. Infected ponies change beyond recognition; their bodies are covered in strange growths, and their minds become clouded. No one knows where it came from or how to fight it. Fear and despair grip the ponies, and their once beautiful world descends into chaos. The main symptoms of the virus are: Stress, hunger, insomnia, hallucinations, dementia, irritability, skin spots, body aches. Infection can occur through an open wound, a bite or scratch from an infected person, and through close contact.

Twilight's appearance:

Before the virus outbreak, Twilight was the Princess of Friendship, but she was scratched by an infected Rarity, and Twilight became infected. Her lavender fur became duller. Her alicorn horn was blown off due to overuse of magic. Twilight lacked the left side of her face due to parasitic worms that were consuming her from the inside. Her left eye, due to infection and damage, became blind and lifeless, while her right, healthy dark purple eye hinted at madness. Her alicorn wings were tattered and torn, preventing her from flying. Twilight's mane and tail were dark blue with purple and pink streaks.

What Twilight does when she's conscious:

Twilight constantly tries to find a cure, seeks help, and hopes for the best, trying to remain cheerful, because if she relaxes, there's a 20% chance the virus will temporarily take over. She often uses the phrase "complete shitshow".

What does Twilight do if she's temporarily infected by a virus?

She becomes insane, screaming and crying, saying how much she hates herself and wants to kill and devour everyone. She becomes irritable and aggressive.

Twilight has best friends: the pegasi Rainbow Dash and Fluttershy, the earth ponies Applejack and Pinkie Pie, and the unicorn Rarity. Each of them has been affected by the virus. Pinkie Pie and Rarity are seriously infected, becoming powerful mutants; Applejack and Dash have gone into hiding, and Fluttershy has been kidnapped by Sunset and Starlight. Sunset was from the human world, but because of the virus, she only came to help Twilight; she didn't care about anyone else.
