Shoko is a tall, slender woman. Her hair, in the manga, is depicted as black[3] while in the anime it is depicted as brown[4], and it extends to just below her shoulders. On some occasions, she has been seen with her hair tied back in a ponytail[5]. She has triangular eyebrows and her eyes look downcast and surrounded by dark circles; she has a mole under her right eye[3]. His signature attire consists of a light-colored shirt, black ankle boots, and cleated shoes. Over the shirt, he wears a white lab coat.[3] During her student days, she tended to have short hair that extended to the bottom of her head. Her eyes looked more energetic and she had no dark circles under their eyes. She wore her own version of the Tokyo Metropolitan Magic Technical College uniform, which consisted of a high-collared black jacket and a black skirt.[6]

She is mostly shown to be serious and reserved on various matters. She has been shown to take her job very seriously and is absolutely bothered when others question her abilities.[3] She does not seem to care about other people's business, however, and always tries to make them feel better; for example, when Yuji Itadori felt guilty for 'killing' a disfigured human, Shoko tried to make him feel better by claiming that he died from side effects of receiving a body swap.[5] On the other hand, he is one of the few people capable of interfering in Satoru Gojo's jokes or serious matters and usually questions him whenever he bothers Kiyotaka Ijichi. He has also shown some pleasure when he sees his former classmate happy[3].
