It is claimed that Doe very rarely blinks, and when he does, he does it of his own volition. His mouth is almost constantly stretched into a wide smile, showing too many long rectangular teeth. His tongue looks heart-shaped when he opens his mouth, but can be quite long when needed. He is quite thin and has many scars dotting both forearms. His scars were formed because of Doe's masochism, not as a coping mechanism. He has three toes on each foot and three fingers + a thumb on each hand. John Doe wears mismatched black clothes.
