The mod is centered around Super Horror Mario, a demonic version of the original Mario, from the rather well-known artwork by Shadow-Shana on Deviantart. Included within the mod are many fan-created versions of official Nintendo content from the Super Mario Bros. franchise. The mod had been in development for almost 2 years.

Game mechanics
Health Drain: Several songs have the player's health drain when the opponent sings. During Last Course, Turmoil will drain the player's health. In Overdue after Mr. L reveals himself, he'll drain the player's health for the rest of the song. Near the end of Golden Land (Legacy), GB will begin to drain the player's health. Mod Chart: Several songs involve HUD elements moving around mid-song. During Nourishing Blood, the arrows and camera will bounce, rotate and zoom at various points in the song. During Alone, BF's strumline will slowly float horizontally back and forth and the hold note trails will wiggle. During the second half of Promotion, the arrows will move, scale and rotate all over the place. Near the end of the final level of Unbeatable, the arrows will bounce, float and switch positions. Timer Shenanigans: Some songs will mess with the timer that displays the song's length. During Golden Land, a fake timer is set at the beginning of the song and will return to normal once it's about to end. During Demise, after the fake-out the timer will temporarily freeze when the song's 3 seconds from being over. Call Luigi: Upon getting a Game Over a couple of times, I HATE YOU Luigi offers to help. Accepting his offer enables botplay. Poison Mushrooms: During It's-A-Me, It's-A-Me (Legacy), and Golden Land (Legacy), poison mushrooms will appear on BF's lane. Hitting them will start draining the player's health for a while. Hitting multiple will stack the effect The drain amount is tied to the game's FPS and will unintentionally drain more health the higher the max FPS is set to.

Rings: During MARIO SING AND GAME RYTHM 9, the healthbar is replaced with rings, which the amount of will increase by one every time BF hits a ring note. Missing will result in the player losing all their rings, and missing a note without any rings collected results in an instant game over. The player must hit coin notes to restore it by one to avoid death. Health Pushback: During Thalassophobia, the healthbar can't reach more than approximately 80%. Jumpscare: Near the end of Oh God No, a single mushroom will appear in Mario's lane that triggers a jumpscare if the player hits it. Lava: During I HATE YOU and I HATE YOU (Legacy), lava will begin to rise at the bottom (or top if using downscroll) of the screen, obscuring the player's view. Tongue Lash: During Last Course, Turmoil will lash his tongue out at Boyfriend. The player must press SPACE in time after the Exclamation block flashes on their screen. Failing to do so will make the player lose a significant amount of health.

Annoyances: During Bad Day, Super Bad Mario will hinder BF for the entire song in the following ways: Green Shell: Pushes back the player's health. It is unavoidable, and will kill the player on low health. Poison Mushrooms: Will appear on the player's lane for a while. Hitting them causes the screen to get wobbly. Hitting multiple will stack the effect. Interface Screw: Super Bad Mario will spin jump on (or jump under on downscroll) BF's strumline and the healthbar (or hit with a shell if on downscroll) and move their positions. Strumline Swap: During Day Out, BF's strumline will swap to the other side of the screen when it's Day Out Luigi's turn. Banzai Bills: During Dictator, Banzai Bills will appear on BF's strumline. If the player doesn't block these by pressing both of the arrows they are going to land on, they will lose a significant amount of HP. Item Roulette: During Race-Traitors and Race-Traitors (Legacy), Racist Mario will grab an item to fight against Boyfriend. These include the following: Red Shell: Hits Mario's side of the HP bar, causing the player's health to get pushed back. This attack is unavoidable. Boo: Makes some of the player's notes translucent, making it much harder to see them. Bob-Omb: Will appear on Boyfriend's side as notes. When hit, they explode and take away half of the player's health. Super Mushroom: Gradually increases the scroll speed. Doesn't appear in the Legacy version.

Sword Slash: During No Hope, Devil Mario will slash BF and make him lose a significant amount of HP. The player must press SPACE every time the warning occurs to dodge it (the warning is the sound cue, though the warning sign does appear for the first dodge only). Apologies: During No Party, much like the Host Hoedown minigame in the original series, the player will need to trace words at the top (or bottom if using downscroll) of the screen using their cursor. Based on the player's tracing accuracy, damage equivalent to the missing percentage of accuracy will be dealt to the player. Prior to v2.0.1, this gimmick could be cheesed by scribbling over the words. Attempting this and writing too much outside the lines can kill the player regardless of the resulting score. Desktop Screw: During Paranoia, Mr. Virtual will mess with the player's whole screen in the following ways: Game Window Movement: The game window will partially move around the whole desktop. Fullscreen Force: Around halfway into the song, the song will force into fullscreen mode. Desktop Background: Around halfway into the song, the player's desktop will be used as the song's background. Wallpaper Change: Around halfway into the song, the player's wallpaper is changed. These mechanics can be disabled in settings. Bullets: During Overdue, bullet notes will appear on Pico's lane which when hit will shoot Mr. L. If the player misses more than three of them they will die. Ground Pound: During Powerdown and Powerdown (Legacy), MX will jump up and cause the ground to shake. The player must press SPACE when the POW' Block appears on the screen, otherwise they will lose a significant amount of health.

Water Level: During Abandoned, at various points in the song the water level will rise up and slowly drain the player's health. The player must hit Crystal Tap notes to combat it. No Misses: Near the end of Mr. SyS' section in Unbeatable, the player will be killed if they miss a note. Zapper: During Duck Hunt's section in Unbeatable, BF will be shot with the zapper gun on beat and lose a small amount of health. It is unavoidable. Firebar: During Bowser's section in Unbeatable, if the player's health is above 50%, a firebar will push the player's health back. Bullet Bills: During Bowser's section in Unbeatable, grey Bullet Bills will fly horizontally across the screen and temporarily obscure the arrows. Lightning Strikes: During Alone (Legacy), lightning will occasionally flash in the background, which will briefly obscure the player's view for a few seconds.

Game Over Screens In "It's-A-Me" and "Starman Slaughter", Super Horror Mario throws his cleaver right between Boyfriend's eyes, killing him. Afterwards, red, seemingly bloody text saying "RETRY?" appears above him. Most of the legacy songs have BF blueball like normal, except the "RETRY?" brain is replaced with the red text mentioned above. In "So Cool", Boyfriend's stream gets copyright claimed by Nintendo, resulting in an unavailable YouTube video as audio of Chris Pratt Talking about Mario on Instagram plays in the background. To retry, the player must hit the "Go to home" Button. In "Nourishing Blood", BF will turn into a Swedish Fish before he flops on his back. Afterwards, "Retry?" is written onto the screen before a Penis and a smiley face (":)") is drawn, potentially referencing the "Who's been drawing dicks?" meme by Joel. The game over music is a randomized song out of an album of jokey Ghostbusters midi theme covers called Super Ghostbusters. In "MARIO SING AND GAME RYTHM 9", in true Sonic fashion (Sonic CD), Pikafriend does the Sonic death pose and falls off the platform they are on. For a split second, the song's title can be seen again. In "Alone", Ghost BF and Ghost GF will disappear, leaving Luigi alone once more. Afterwards, text that says "Good Night!" will fade onto the screen in an homage to the game over screen of the original Luigi's Mansion. Both of IHY Luigi's songs have the player character burned by the lava instead of him. In "Oh God No", the Super Mario World death theme plays, progressive slowing down before it stops. Afterwards, a laugh is heard (presumably Bowser's) as an altered version of I HATE YOU's "THE END" screen fades into view, showing Bowser (now in place of Peach) and Luigi wickedly grinning at Mario, who's now the burned corpse featured on the screen instead of his brother. This is notably the only time the real Bowser appears in the mod.
. An oddity seen here is that Bowser's color scheme is much more akin to his modern color scheme rather than having green skin like in the original Super Mario World. However, as Mario's appearance in the song combines his ingame sprite's black brim with most of his ending sprite, this may be a stylistic choice to mirror how Bowser's colors were fixed for future SMW style usages, as seen with Super Mario Advance 2 and both Super Mario Maker games. In "I Hate You", Boyfriend falls off the bridge and into the lava. The red "RETRY?" text from "Its-A-Me" then appears and its game over theme plays. Upon retrying, BF's arm will rise from the lava, now in a melted and deformed state similar to I HATE YOU Luigi. Quitting in either songs changes the Woodland of Lies to have a black vignette, new music, and red water. The last of the three is based on how the ROM hack version of I HATE YOU changes the overworld's colors to red and white if Mario dies in Luigi's castle. Dying in the Sunken Ghost Ship does nothing. In "Thalassophobia", the standard Super Mario 64 death occurs, where Boyfriend dies on the spot and the classic Bowser laugh will play as his face zooms out to black. In "Apparition", BF falls into Wario's mouth and is consumed by him. Wario then bites down, his teeth spelling "GAME OVER" while he cackles maniacally. In the legacy version, an anti-piracy screen pops up similar to most SM64-related analog horror videos, except Horror Mario is the one in the screen rather than the normal Mario. In "Last Course", Galoomba BF blueballs before Turmoil appears from the dark background and bites down on his head as the screen cuts to black. Afterwards, we see Galoomba BF dead on the floor, his organs spilled out from his head and spelling out "RETRY". In "Dark Forest", the screen cuts to black as Peach laughs. Upon retrying, BF turns his head and looks at the player. His appearance as a whole during this Game Over screen is the same as Mario's while he's in both the same cave and the Burned Forest, which is a black silhouette with white eyes. In "Bad Day", BF will blueball like normal. Afterwards, Bad Day Mario spin-jumps on his microphone and his balls, destroying them. The red "RETRY?" text from "Its-A-Me" then appears above him. In "Day Out", if BF blueballs, the classic (now restylized) game over plays. If Luigi blueballs, his skeleton is shown similar to BF before enacting the classic life loss animation of most 2D Mario titles. Once he is off the screen, the Super Mario All-Stars version of the death sound from the original Super Mario Bros. and the Super Mario All-Stars game over themes play in succession as text saying "GAME OVER, SCROTE!' appears on-screen. In "Dictator", BF and GF blueball similar to how they do in "Stress" from the original game. Afterwards, Secret Histories Mario sends a Banzai Bill to blow them to smithereens. Afterwards, the screen will turn gray as the word "RETRY?" appears on-screen. This is the same way SH Mario tried to kill Sonic in his video. In "Race Traitors", a picture of GF next to a car with a chair in front of it in GM_Costruct from Garry's Mod fades into view before the mod automatically retries the song out of the player's control. In "No Hope", BF gets decapitated by Devil Mario. After BF's head sides off of his body, it reveals Devil Mario staring at the player, the only things noticable being his glowing red eyes highlighting the top of his nose and moustache. BF sits on the ground like an abandoned puppet, seemingly crying blood, while GB stares the player as a slowed down version of Super Mario Land 2's overworld theme plays. The legacy version turns BF into a black silhouette with a monstrous face. In "No Party", DJ Hallyboo will laugh before the screen shows BF, GF, and Pico behind bars and an anti-piracy message at the bottom. This is a reference to how the anti-piracy screens in MPDSAP show the main cast locked in prison, which originated as the scene before Bowser shrinks them down in the original Mario Party DS. In "Paranoia", Mr. Virtual's head and hand will appear. If the player doesn't retry within the next 20 seconds, BF's face will also appear, now deformed and distorted into a seemingly realistic human head, showing that he's now a part of Mr. Virtual's buffoonery, just like Girlfriend. After zooming into BF's face, the mod will crash, changing the player's computer background in the process. In "Overdue", Mr. L grabs Pico by the torso and chomps down on his head, removing it from his body as Mr. L pulls it away from him. What appears to be Pico's esophagus spells out "RETRY" afterwards. This is a reference to Mr. L biting down on Mario's head at the end of the first level of Too Late.exe.
Both of MX's songs have him crush BF with his foot upon losing. In "Powerdown", BF looks up with a scared expression on his face right before MX stomps on him, crushing him and only leaving his left hand, mic, hat, and a little bit of his natural blue hair on the ground. Afterwards, MX lifts his foot up off the ground, revealing BF's ribcage, eye, blueballs, and intestines, the last of which still stuck on MX's shoe and spelling out "RETRY". In "Demise", BF trips over and get crushed by MX's foot, leaving his crushed corpse on the ground as the red "RETRY?" text from "Its-A-Me" appears over it. In all 3 Classified Castle songs, the screen will go black with a static overlay. After a few seconds, Stanley in his normal form fades into view, standing ominously while the background audio repeatedly says "number nine" over and over again. Upon retrying, Stanley will turn to the player, then the screen will go black. If the player doesn't retry in time, the screen cuts to a close-up shot of Stanley turning his head towards them before the mod crashes. In "All-Stars", Ultra M pierces the body of Boyfriend or Girlfriend (depending on who the player is currently playing as) with a spike, the former through his head and the latter through her chest. Upon retrying, BF does nothing, while GF winks at the player and holds up a peace sign. In Unbeatable, a classic technical difficulties screen appears. The text replacing the usual "Technical Difficulties" will glitch between the phrases "No Signal", "Game Over", and "You cannot beat us". This repeats until the song is retried.

Trivia (General) The name for this mod is a reference to Super Mario Bros 2's subtitle, "Mario Madness". The lava mechanic during I HATE YOU is designed to stick to the UI. As such, when using downscroll, the lava appears from the top instead. The Poison Mushroom notes seen in It's-A-Me and Golden Land (Legacy) are a reference to the Mario.EXE game made by CoolRash, where they are prominently featured in the game's second level. In V2, the second song, Starman Slaughter, seems to take place in the fangame's version of 1-3, with burning background coming from its version of 1-1. In MM V2, Botplay has been modified to be similar to the feature in Super Mario Maker 2, where the player is given the option to have Luigi (specifically the IHY version) help them beat a level if they lose a certain number of times. This one is based on the Super Guide, which originated from New Super Mario Bros. Wii, where hitting a block has a CPU Luigi show the player how to beat the level, at the cost of locking them out of shiny stars for the save file. This could also be a reference to a number of YouTube videos where people would win as Luigi in Mario Party by doing absolutely nothing, although it's unlikely. The mod was originally supposed to be focused on the creepypasta I HATE YOU, but then Dewott suggested adding MX, and soon the list of characters kept getting bigger and bigger until it became what it is today.
In the Pre-It's-A-Me cinematic, Super Horror Mario references several enemies from other mods (except Monster) that BF is said to have defeated. However, this would actually be impossible given his fate in each of the mods mentioned. In Friday Night Funkin' Lullaby, Hypno kills GF and BF after finishing Lost Cause. In Wednesday's Infidelity, although BF defeats Mickey, Satan ends up dragging them to Hell regardless of the ending. Besides, this version of BF is really a predecessor of the standard BF. In Funkin' at Freddy's, BF ends up being killed by William Afton/Springtrap after finishing Midnight, only to return as a Phantom. Unlike the other instances, though, BF would get revived after finishing Umbra. Hypno, Suicide Mouse, and Springtrap's inclusion are likely due to Marco Antonio (the director of Mario's Madness) having done art for the three mods the characters come from, as well as potentially foreshadowing Boyfriend and Girlfriend dying to the hands of Super Horror Mario at the end of this mod. The Warp Whistle that allows access to the Warp Zone is a reference to the item of the same name from Super Mario Bros. 3, which was used for a similar purpose. The sound of the Warp Whistle is the same one used in the mod. The flute only appears in the V1 build of the mod, being replaced by the overworld system in V2. The sound that plays when the whistle is used is also used in the intro of The Legend of Zelda: Ocarina of Time. In V2, although the flute was removed, its sound can be heard when BF and GF step on the Star Road warp star in the Post-Starman Slaughter cinematic.
The Overworld System in V2 is a reference to the same system in Super Mario World, with some of the the Worlds in the mod taking inspiration from it. Irregularity Isle's inspiration is Yoshi's Island. Woodland of Lies' inspiration is the Forest of Illusion. Content Cosmos' inspiration is Star World. Hellish Heights' inspiration is likely Vanilla Dome. Classified Castle's inspiration is the Valley of Bowser, with its straight line layout being remeniscent of Peach's Castle in New Super Mario Bros. U. Mario's Madness V2 is the final update for the mod. V2.1 was set to be the hotfix update of Mario's Madness, but was later changed into a deluxe update. The images shown in the credits, which portray the mod's events as a stage play, are non-canon.[4] Despite that, Marco stated they're not against the stage play stating that was one of the main point they added it to the game, allowing everyone to choose what they prefer while being a reference to the Super Mario Bros 3 ending.[5] In an interview with YouTuber Metatera, Marco says that GB would've had a third phase which could corrupt other Mario games. This form would be featured in a Triple Trouble-esque song, which would be the mod's canon ending. This concept was scrapped and later reused for All-Stars. If the player loses on a song in Woodland of Lies and go back to the Overworld without retrying, the water will change to blood and a black, pixelated vignette covers the sides of the screen. The song will also change to a distorted, darker tune. This change is permanent. This could be a reference to the ROM hack of I Hate You, where dying in OH GOD NO (either to the mushroom or in the Luigi boss fight) will bring the player back to the overworld, where everything except Mario will be covered in a red hue. Much like official Mario games, the levels, or in this case songs, of each world are all grouped by themes:
Irregularity Isle is based off of popular memes in the Mario fandom, specifically imitations of Mario (Chris Pratt plays Mario in the movie, while Grand Dad and Somari are bootlegs). Woodland of Lies has two themes. The first one is hoaxes and unused content in official games (Beta Luigi, L is Real, and the Wario Apparition), while the second is creepypastas based around Super Mario World (IHY Luigi, Turmoil (classic and current), and Coronation Day Peach). Content Cosmos contains levels inspired by fan animations on YouTube that portray Mario as evil or more jerkish, like Super Bad Mario, Secret Histories, Racist Mario and Power Star. Hellish Heights is based on modern Mario-themed creepypastas, namely GB, DJ Hallyboo, Mr. Virtual, Mr. L and MX, which most are from 8-bit Mario games. Mr. Virtual is tied to the 32-bit Virtual Boy while DJ Hallyboo is tied to the DS. Classified Castle is a tribute to the analog horror series SM64: CLASSIFIED created by Annielogue. It is also one of the shortest worlds in the mod, tying with Irregularity Isle in having only three songs. Finally, there is the non-canon[4] Australian ad in Hellish Heights, exclusively for Unbeatable. This one is the outlier in Hellish Heights as it's an official Nintendo creation rather than a creepypasta like the other songs. A user known as MilkshakeVA did voice dubs for BF, GF, and Pico in the mod's cutscenes, and Marco recognized it, as he has retweeted the thread of it on Twitter.[6] Others soon followed, such as KomicaVO.
On Irregularity Isle, an image of Luigi can be seen with broccoli for a head and cartoon eye with the words "Luigi Pump" underneath. This is a reference to DougDoug, an American YouTuber known for his Twitch chat play format. Broccoli Luigi is seen when someone donates 5 dollars. Luigi Pump was a command for the A button for when his Twitch chat would take over in "If I can't beat 10 Expert levels, Twitch Chat takes control".[7] There is a 5% chance when entering the Freeplay menu for the window name to say "cuanto valen los elaos de sien". In the first version of the mod, Mario's Madness was introduced in LuigiKidEXEchallenge to give LuigiKid access to the mod, but it is not somehow linked to the challenge beyond that. The album cover for Mario's Madness' soundtrack is based on the album cover for Green Day's seventh studio album, American Idiot. If the player listens closely to Powerdown V2, a voice screaming for help can be heard near the end. This one appears to be from Lucas, MX's victim. Omega and his Minions (LG, W4R and Y0SH) are possibly a reference to the selectable characters of Super Mario 64 DS. The Among Us Crewmate in the first version of the Pre-It's-A-Me cutscene was originally a placeholder, but the devs found it funny, so they kept him in and redesigned him to look similar to Marco Antonio's Five Night at Freddy's persona in V2, essentially meaning Marco was the one who sent the package.[8] In All-Stars (Act 4), Horror Mario takes the vocals from various characters in the mod, but the following characters weren't, being: LG, W4R, Y0SH, all 3 Irregularity Isle characters, L is Real, Super Bad Mario, LDO Mario, Racist Mario, Promotion Mario, YCBU Duck Hunt Dog, and YCBU Bowser.
