Ciel is a short, slight teenage boy with grey-tinged, blue-black hair and rich peacock-blue eyes. As described by his tailor, Nina Hopkins, Ciel has a well-proportioned body: His arms and legs are thin, his shoulders slim, and his waist narrow. On the left side of his body is the mark of the "noble beast", which he was branded with during his time as a slave in a cult. Around this time, his slave-traders also pierced his ears to attach a price tag.[16] Since then, he wears earrings. Ciel typically dresses in a way that suits his noble standing, and he has an extensive wardrobe. He almost always wears a black eye-patch made of gauze with a single cord over his right eye to hide the location of his Faustian contract with Sebastian Michaelis. When in disguise, he trades the black eye-patch for a white patch that resembles a transdermal patch, which is fastened over his ear with two cords. Ciel sports two rings. The one on his left thumb is an ornate silver piece that holds an emerald-cut deep-blue sapphire; it is a one-of-a-kind family heirloom that has been passed down for generations. The one on his right hand is a gold signet ring in the form of the Phantomhive crest, which he uses to stamp the wax seal on documents. Upon his return, the ring was given to him by Angelina Dalles, who told him that it was the only ring that had not been destroyed in the fire that killed his parents.

On December 14, 1875, two newborn twins were born to Vincent and Rachel Dalles-Phantomhive, Ciel and an unknown named baby, who let's just call the spare, since he was called one during his life. Their aunt Angelina Dalles helped to deliver them. On their tenth birthday, Ciel was woken up by the spare and given one of the two rabbit fèves from their advent calendar. They wished each other a happy birthday before they got dressed and went downstairs to marvel at the decorated dining room and the presents under the tree. The twins were then congratulated by their parents and given the day off from their studies. Happily, Ciel and the spare walked around the manor until they arrived at the kitchen. There, Tanaka and the cooks shooed them away as they were not allowed to see their birthday cake until the party started. Tanaka told the twins to go to their room and that he would come to get them ready for the party at six o'clock. Ciel and the spare went to spend the time waiting by playing games in their room and fell asleep after a while. When they woke up, it was already six o'clock, but Tanaka still had not arrived. They continued to wait, and when there was still no sign of Tanaka 45 minutes later, the twins became anxious, and Ciel decided to go and find out what was wrong. He refused his brother's wish to accompany him and told him to stay in their room before he left. Ciel wandered around Phantomhive Manor in search of answers. Eventually, he discovered his parents' corpses and retrieved the blue Phantomhive ring from his father's finger. He called for help and ran into Tanaka, who urged the young boy to flee. Before he could say more, however, Tanaka was attacked from behind. At some point during the attack on the manor, something terrible happened to Ciel. Alongside the spare, he was later kidnapped by human traffickers and tagged. The twins were put in the same cage, and Ciel apologized to the spare for being pathetic and "a failure of an older brother" as he had not been strong and powerful enough to protect him. In their misery, the brothers began to cry and embraced each other. At night, Ciel woke up his brother to show him the blue Phantomhive ring and tell him that it would help them return home. Then, Ciel swallowed the ring to ensure that it would not be taken away from them. The next day, the twins were brought by a man and taken to his mansion. There they were washed, dressed, and given food. While the spare believed that they had been saved by a "nice man," Ciel remained suspicious. Later on that day, the twins were dressed for a "party." The man took them to a cathedral where a cult consisting of people wearing masks in the shape of moths had gathered. The spare asked the man, who had brought them there, where they were, but he only replied that they did not have to be afraid before announcing to the crowd that the twins would be their next sacrificial lambs and that they had to defile their pure and innocent souls in order to "prepare" them for the ritual. Afterwards on the day before Christmas, Ciel and the spare were branded with the "mark of the beast." In the following month, the twins were continuously violated, interjected with an unknown substance, force-fed, and abused in various different ways. However, as they were together, they did not completely lose hope; and Ciel declared that he would always protect the spare. At the end of this month, Ciel was used as a sacrifice in a ritual to summon a demon while the spare had to watch helplessly. In the process of the sacrifice, Ciel was stabbed in the chest and died. Fully embracing that there was no God coming to save them, the ritual was successful. However, the summoned demon unexpectedly chose to form a contract with Ciel's twin (instead of with any of the cultists.) The demon, he later named “Sebastian Michaelis.” He appeared in his true form and told the spare that what he had sacrificed will never return. Sebastian asked the spare his name, and if he wanted to make a contract and have his wishes granted. The spare declared that his name was “Ciel Phantomhive,” even though it wasn't, and that he wanted the power to take revenge on those who mistreated him. The cultists tried to stop him from talking, but the spare and Sebastian forged their contract. Sebastian asked him where he wanted his seal, saying that the more visible it is, the stronger the contract is. The spare told him that anywhere was fine and that he wants a power stronger than anyone else's; Sebastian called him greedy and decided to put it on his right eye, causing the spare to scream in excruciating pain. He then commanded him to kill his attackers; Upon the spare's request, the demon killed the cultists and recovered the blue Phantomhive ring from Ciel's body before they left and set the cathedral on fire. Finally, the spare stated his absolute orders: Sebastian must protect and never betray him, to obey his orders no matter what, and never to lie to him. The first place they went after these events was to the Royal London Hospital where his aunt worked; he was reunited with Tanaka, and a carriage was arranged to bring him back to his manor. He and Sebastian viewed the ruined manor, then visited the graves of him and his parents. Sebastian left briefly, and when the spare returned to him, he found the manor had been rebuilt by the demon. The spare later took his place as the Earl of Phantomhive and the Queen's Watchdog. He established Funtom Corporation in the same year, fulfilling his childhood dream of opening a toy store in London. A year later, the spare summoned Sebastian to his room, complaining of a wobbly tooth that was preventing him from eating his food properly. After examining it, Sebastian decided to forcibly take it out, much to the spare's chagrin. The spare attempted to protest the idea but Sebastian proceeded regardless, under the impression that he was doing his young master a favor. After the tooth's removal, the spare threw an angry fit at Sebastian. Ever since he has been unwilling to allow Sebastian to check for cavities in his teeth when they start to wobble. Sebastian endeavored to recruit servants for the spare. After successfully recruiting gardener Finnian and housemaid Mey-Rin while house steward Tanaka was on leave, Sebastian introduced Baldroy to the spare as an American soldier who will be serving as chef. Forever posing as his elder twin brother, “Ciel Phantomhive.” And has been doing that for three years now.

Memory One: Black Butler Arc
One morning, Sebastian Michaelis serves Ciel Phantomhive a refined breakfast, dresses him, and updates him on the day's schedule. Sebastian’s skills are showcased throughout the day, and Ciel learns that Chlaus from Italy will arrive at six. Sebastian promises to host him well. When Chlaus arrives, Ciel requests his usual goods, and Chlaus hands over a rare "game," though Ciel asks for more, noting kids' greed for games. During the evening meal, Mey-Rin accidentally spills wine, but Sebastian swiftly clears the tablecloth without disturbing the meal. Ciel tells Chlaus to ignore the mishap, and they enjoy dessert. Memory Six: Phantomhive Manor Murders Arc
When Charles Grey and Charles Phipps arrive at Phantomhive Manor, Grey attacks Baldroy and Mey-Rin. Sebastian intervenes to save Baldroy and then lets them meet Ciel. They inform Ciel that Queen Victoria wants him to host a banquet, which Ciel reluctantly agrees to, aiming to regain her trust. He instructs Sebastian to prepare and contact Lau and Undertaker. On the day of the banquet, Ciel talks to Diedrich, who warns him not to overdo it. Sebastian informs Ciel of the arriving guests and a brewing storm. Ciel greets the guests, including Georg von Siemens, and encourages mingling. When Georg throws a drink at Irene Diaz, Ciel steps in. After Sebastian handles the guests, he dries Ciel and they insult Georg in French. Later, Georg passes out, and Ciel orders Sebastian to take him to his room before heading to bed. He lets his butler talk with Sebastian but tells him to settle the matter in ten minutes. However, Claude later returns with Sebastian alive and Alois opts to punish Claude but the latter refuses. Soon after, Alois angers Ciel by dancing with Elizabeth Midford. He vows to "have" Ciel, while Ciel vows to kill him. Consequently, he duels with Ciel. He corners Ciel at first, but as the butlers arrive, he gets stabbed in his stomach and bleeds profusely. Alois is mocked by Ciel, who believes that Alois killed Ciel's parents. Alois tries to explain, telling Ciel that they are both the same as they are being deceived by a demon. He begs and cries as he holds onto Ciel's leg. Ciel is about to strike him dead when Claude finally stops him. In separating them, Claude inadvertently tastes Ciel's blood and nearly swoons in ecstasy. He is too focused on the taste of Ciel's blood to help the wounded Alois, who vomits a gush of blood before passing out. The ring Alois wore in life, which also contains his soul, is now being worn on Ciel's finger and his memories have been mixed in with Ciel's. He soon takes control of Ciel's body and climbs to a clock tower in front of the mansion.[6] Alois then creates a maze in which Sebastian and Claude are asked questions in order to get to Ciel's body. Claude has an advantage because of his knowledge of Alois. Ciel's soul briefly breaks through and orders Sebastian to remain his butler until he has consumed his soul entirely, but Alois once again takes control of Ciel's body. Alois states how much he was attached to Claude and allows Claude to get every answer right in the maze of his heart. Upon learning that Claude murdered him in order to get to Ciel, Alois has an emotional breakdown. Hannah reveals that Luka, his brother, had contracted to her in order to destroy the village he and Alois grew up in for Alois's sake. Alois talks to Ciel in his subconscious thoughts as they wait for their butlers to finish dueling. It is later revealed that his wish he made with Hannah was for Ciel's soul to never get eaten, and for Alois to join Luka in Hannah's body.

A Bizarre Doll is a carnivorous being who preys after the souls of humans. It is an erroneous experiment of the Aurora Society's medicine, and is meant to be resuscitated back to life instead of becoming a soulless, moving corpse. The rightful creator of the Bizarre Dolls is the Undertaker and, in order to resuscitate them, he adds fake records to their Cinematic Records. - Later, the Bizarre Dolls are advanced: When they die, humans do not only recall their pasts, they also carve and mourn the futures they could have had. Undertaker has found a method to extract those episodes of longing and desire and attach them, instead of the fake records, to people's Cinematic Records. By doing so, the resulting Bizarre Doll will be "infinitely approaching a living human" as they run on their own "future forecast." However, the current success rate is rather low as it is dependent on the quantity and quality of the extracted episodes. The Bizarre Dolls created with this new method are often indistinguishable from humans as they retain their consciousness, senses, and ability to speak; they are also not driven by the instinct to obtain a soul anymore. These Bizarre Dolls are able to fool demons as well, though they might feel a slight sense of discomfort in their presence. However, even the evolved Bizarre Dolls cannot maintain their biological functions, i.a. their circulatory systems, as they are still nothing but reanimated corpses. They, thus, need regular blood transfusions to make them function. If Undertaker were to fix this problem though, Bizarre Dolls would be "practically the same as living, breathing humans." This potential development would cause the definition of "souls" to crumble in this world. As noted by Sebastian Michaelis, a body of a Bizarre Doll is considerably softer than a regular human being.
