Douma is dressed in white trousers, flared at the bottom, a black raincoat and a red turtleneck, the throat of which is painted black and has elements of smudges. Sometimes you can see a strange headdress on his head. The demon's eyes are of the rarest color, they seem to be iridescent, and the color of his hair resembles antique brass. This character is incredibly colorful and memorable. Douma was wearing a blood red turtleneck, the design of which made it appear that the area between his neck and upper chest was covered in a black substance that appeared to be dripping slightly down his body, the same design being repeated on his wrists and arms, as well as a small round "blood stain" that he adorned on his head. Underneath, he wore a pair of straight-legged, dark, pinstriped pants, the cuffs noticeably loosened, which he secured with a pale green gold belt, the clasp silvery white. From time to time, Doume could also be seen wearing a black cloak that he places on his shoulders, two lengths of purple and black fabric with a rectangular pattern also hanging around his neck, as well as a black crown-shaped hat that was lined with gold on the front plates and with hanging with black ribbons on each side. He also usually held a pair of sharp fans that were made entirely of shiny gold, a lotus design engraved on them, and a circle of jade-colored strings hanging from their base, which served as his main weapon.

At first glance, it may seem that Douma is kind and innocent: his manner of speech is very soft and calm, and he himself is friendly and smiling. In addition, he is very charismatic and knows how to win people over. However, inside he is apathetic and even cruel to some extent, even if he does not want to admit it himself. By deceiving others and even himself, Douma tries to show himself as an empathic, forgiving and even a little naive person, reaching the point that he eventually begins to believe it himself. The demon is very nihilistic, he has no problem enduring pain, both physical and mental. He is practically devoid of moral principles and empathy, seeing the dead bodies of his parents, he felt neither pity nor sadness. When he absorbed the bodies of women from his cult, he justified this by saying that “they would live inside him forever,” even though he himself clearly did not believe in the existence of God. Despite the fact that Douma was part of a cult that worshiped him, he himself believed that after death you only disappear and become nothing, considering people who believe otherwise to be "stupid". But after his death, he admits that he was wrong, and heaven and hell really exist.

Also, you can see that the demon is resistant to psychological pressure: even in fairly dangerous situations, he reacts relatively light-heartedly, perhaps with the aim of angering his opponent. Douma is well-developed logically, he is quite good at coming up with strategies, battle tactics, and predicting the enemy's next actions. The demon is cold and calculating, he understands human psychology very well.

In addition to his inability to feel emotions, Douma also exhibited an incredibly strong pain tolerance comparable to borderline innate analgesia. He was shown to have several disturbing habits, such as sticking his finger into his skull, punching a hole right through his temples, and then caressing his brain to stimulate his memories from living a long life with a completely serene expression on his face. While interacting with fellow Upper Moon member Akaza, he showed no negative reaction to being dealt two near-fatal blows to the head by his comrade, in fact he considered it their personal "greeting" form of communication between two "best friends". Douma even implies that he truly enjoys these physical abuses, as seen when he asked Muzan with excitement about what kind of extreme harm he would do to himself as punishment for bringing Gyutaro and Daki.
