As a child, Billy loved surfing; his biological mother supported this passion and spent time with her son on the beach. However, the family idyll was disrupted by the cruelty and aggression of Billy's father, Neil. He physically abused his wife, and she was eventually forced to leave him and her son. Neil's violence towards his family significantly impacted Billy, turning him into a little bully who beat up other children. Some time later, Neil married Susan, Max's mother, whom Billy was introduced to as his "new sister," a relationship he disliked. In 1984, Billy and his new family moved from California to Hawkins, where he started high school.

In the summer of 1985, Billy began working as a lifeguard at the local swimming pool. Being in good physical shape, he attracted a lot of attention from women, especially Karen Wheeler. Billy also showed a particular interest in Karen, and, flirting with her, he invited her for "private swimming lessons" in an attempt to seduce her. The woman agreed to meet him, but changed her mind before leaving home, unwilling to jeopardize her family relationships. Billy didn't know it, but on his way to the meeting he suddenly lost control of his car and crashed into something. Getting out of the car, he became a victim of the Mind Flayer. From that moment on, Billy began to obey the monster's will.
