Kanae was a young, slender, average-height girl who bore some resemblance to her sister Kanao.[3] She had black hair, which she usually wore loose and extended down the length of her back.[4][5] She also had bangs parted on two sides across her forehead and two long locks hanging down from her temples. She had lilac-colored eyes and her lips were painted red.[6] She wore a double hair ornament shaped like butterfly wings[6]; on one side of her head, she had a green and pink ornament in the anime[7], and a red and lavender one in the manga. She wore the Demon Slayer uniform with no visible customization and wore a haori that had a butterfly wing design[5].

Kanae had a carefree and kind nature, showing no signs of anger or annoyance when speaking, appearing to be the less mature of the two sisters despite being the older one[8]. In Kanao and Shinobu's memories, Kanae used to smile constantly[8]. However, when Sanemi Shinazugawa was about to unleash his anger on Kagaya Ubuyashiki for the death of his friend, she agreed to calm him down by telling him that Ubuyashiki never forgets the names of those Demon Slayers who were killed, demonstrating how much she cares for the Demon Slayers under her responsibility[9]. She also had a dream: to see a world in which humans and Demons coexisted peacefully despite all the evils and tragedies caused by the latter. Despite this, she also had a much stricter side than Shinobu's, appearing as a spirit to the latter to encourage her to fight Doma, leaving behind any excuses or weaknesses[5]. She was very observant, noticing Doma's lies and actions; according to Kanao, she felt pity and compassion for the demon's personality, even though he was the direct cause of her death[10].

The Kocho sisters lived a peaceful and privileged life with their wealthy and loving parents, who continually adored and cared for them. Growing up lacking nothing, the two sisters were content with their simple life together. However, this all came crashing down when a demon attacked their home. Their parents attempted to protect the two, but were slaughtered before their daughters' eyes. They would have been next, but were saved by the timely arrival of Gyomei Himejima, who killed the demon immediately before it could kill either of them. Following these traumatic events, the two sisters made a promise to join the Demon Slayer Corps to prevent the demons from inflicting the same horror and pain they experienced on others. [7] They both soon became successful Demon Slayers after completing their training and developing their own Breathing Styles. At some point, Kanae became the Flower Hashira. She was present at Sanemi's first Hashira meeting, looking on in alarm when he roughly confronted their leader. When Kagaya mentions Masachika Kumeno by name, Kanae informs the shocked Sanemi that Kagaya had taken pains to memorize the names and backgrounds of every Demon Slayer who died in combat. One day, while traveling through a village, Kanae noticed a small girl being dragged by a man on a leash. Though the girl was nameless, dirty, and nonverbal, Kanae approached and attempted to convince the man who had purchased her to let her go. When he demanded money as compensation, Shinobu tossed the money in the air, grabbed the rope he released, and fled with the girl. She was then taken to the Butterfly Mansion, adopted, and named Kanao. When the girl stopped responding without commands, Kanae gave her a coin to toss and help her make decisions until she could do it on her own.
