However, the deaths were not accidents, but merely cover-ups so that Cioccolata could operate freely, diagnosing healthy patients before doing so. He would sometimes limit the anesthetics so that patients would regain consciousness mid-operation, allowing him to delight in the patient's suffering and death. From a distance, Cioccolata is extremely cruel, as well as killing patients and emotionally abusing elders into committing suicide. The addition of his stand, Green Day, highlights his callous nature - infecting anyone with his flesh-eating mold without remorse. Even Diavolo is reluctant to dispatch him to get rid of Bucciarati's group, and he is considered a monster by the members of Passione.
Cioccolata is driven by his morbid curiosity, insisting on recording every death he witnesses and enjoying every moment, as he claims it is for his personal advancement as a surgeon. His sadism gives him enough cruelty to put Giorno and Mista in danger. Cioccolata also has a tendency to boast, presenting his philosophy of his own advancement at the first opportunity and loudly expressing his satisfaction at the suffering of others. Near the end of the fight, Cioccolata reveals to Giorno that he believes in a theory of happiness. The mold's growth is triggered by decreasing altitude and has unlimited range as long as other people are infected. Cioccolata can safely dismember and reassemble itself Surgical Genius: Cioccolata is a trained surgeon and his sick experiments have given him an intimate knowledge of the human body, notably which body parts he can cut off while preserving the life of his victim. His surgical skills are great, as he could perform surgery on himself quickly enough to split his own body apart while Mista and Giorno climbed up to find him. Additionally, his knowledge of medical tools and sealing blood vessels allows him to limit his blood loss so that he does not bleed out during his battle with Giorno. While in the original manga, Cioccolata simply puts himself back together afterwards, the anime adaptation expands on this. Instead, Cioccolata is shown stitching himself back together.
Cioccolata, at age 14, mentally abuses the elderly A young Cioccolata was constantly at the top of his class. At the age of fourteen, he volunteered to care for bedridden elderly people and received a commendation from the town mayor himself. In reality, however, Cioccolata did not care for the elderly. Instead, he intentionally fed them unsanitary food and illegal drugs, and then whispered in their ears about how no one visited them and other emotionally destructive things. This torment eventually drove the elderly to despair and eventually to suicide. He even filmed the expressions of his helpless victims and their deaths. Diavolo discovered that he had kept over 25 tapes of this material. After psychologically manipulating nine patients into committing suicide, Cioccolata decided to become a doctor.
Cioccolata as surgeon Cioccolata eventually became a surgeon, continuing his crimes on unwitting patients. There are four cases where Cioccolata misdiagnosed people for surgery, but also botched the anesthetic dosage so that they woke up mid-surgery. Two years before the events of Vento Aureo, Cioccolata killed one of his patients. He was caught and fired, but his deliberate murder was mistaken as an accident. The anime version has the patient having his stomach cut open and his entrails touched by Cioccolata. At some point, one of his alleged victims, Secco, was somehow spared by Cioccolata and ended up as his underling and a sort of "pet". Eventually, the two would join the underground crime syndicate Passione and gain their Stands, though the Boss believed their abilities were too dangerous to carry out missions and that their loyalty was fickle. The anime suggests that he, alongside Secco, was the one who brutally cut Sorbet of La Squadra Esecuzioni into pieces while a helpless and captured Gelato choked on his gag.
He was first introduced when Diavolo, explaining to Doppio, stated that he had no choice but to call Cioccolata and Secco to intercept Team Bucciarati in Rome. (In the anime, he makes a brief appearance in Squadra Esecuzioni's backstory before brutally executing Sorbet and Gelato.) Diavolo, while not very hateful or spiteful towards many people, hated Cioccolata, referring to him as "despicable" and "trash." He is first seen using his Stand Green Day on Mista and Narancia. With his Stand in effect, he moves to a safer location above the group as they suffer, telling Secco to record their pain. When Mista, Bucciarati, and Giorno escape their death trap, he and Secco board a helicopter to climb above Rome and increase the effects of Green Day, throwing corpses with the mold directly at Bucciarati and company's car. Secco jumps out of the helicopter to stop the gang, but ends up fighting Bucciarati while the other two head for Cioccolata's helicopter.

In the light novel, Cioccolata has a counterpart in the 37th universe as a criminal wanted by Interpol, the CIA, and even the FBI. He works for Passione selling drugs and is involved in terrorist activities and, like his original counterpart, has a partner named Secco Rotario. He wields a Stand called Blob, which could create tornadoes according to NYPD Blue.
