Deku is the only extra personality he has, its duration is unknown but he lasts longer "conscious" than Izuku. He developed this phenomenon when he was 14 years old. Neither of them controls anyone else, it's like a slight migraine in the head that changes them, they are different people in the same body and it is uncontrollable. Neither of them remembers what the other does, and the only thing they know is that they have to live together in 'harmony' (it is an incurable disease).
