Reaper Nurses are thin, fair-skinned female humanoids dressed in 20th century nurse attire. Their faces are apparently hidden by what appears to be a white paper bag with a black "plus" sign, the first aid symbol, emblazoned on the front, though they can still see. Like the Matron, Reaper Nurses wear red combs over their bags. While it is unknown what their faces look like, one piece of merchandise art shows them wearing pink lipstick and the mouth and chin of a normal human. Their attire consists of a tight red and white rubber/latex dress with red buttons on the shirt and skirt. They wear white leggings and have black first aid symbols as accessories on their shoulders and belt. They wear short red gloves and red Mary Jane-style heels with wheels attached to them. Their most noticeable feature is a large syringe that they use to kill their victims. The Reaper Nurses have two weapons: a giant syringe, which serves as their primary weapon, and a pill.
